Feedback and Complaints

We welcome feedback from anyone who has been in contact with VITAL, whether you have used our services, a family member of someone who has, or a professional or other interested person.

To leave us some feedback just click on the Feedback button below. If you want to make a complaint, the full policy is available here, and you can fill in the form online. For further support around making complaints, please contact the office to speak with someone directly.

We would ask that prior to making a complaint, you contact us via phone or email and explain the issues that we might be able to resolve quickly and informally.
